Penn-York provides a number of services, most of which fall under the categories of Community Support Services, Employment Services, Family Services.
This service is a community-based program focused on social inclusion, building interest, developing skills and potential for competitive integrated employment.
Respite services are put in place to supervise and support participants living in private homes on a short-term basis for planned and emergency situations, giving caregivers a period of relief.
Companion services are for participants who are 18 years of age or older who live in a private home for the purpose of providing supervision or assistances with health, safety and welfare, ADLs supports and to promote a meaningful home and community life.
Providing transportation to and from programs or appointments. Transportation is a direct service that enables participants to access services and activities specified in their approved service plan. This service does not include transportation that is an integral part of the provision of another discrete Waiver service.
This service is provided in a home or community setting to assist participants in acquiring, maintaining and improving the skills necessary to live more independently and participate in a meaningful community life.
Supported living services are provided for participants who live in a private home that is owned, leased or rented by the participant. This service is to promote the health and welfare of the participant.
This service in intended for participants who may need assistance acquiring skills to replace problem behaviors that affect their family or community life.
This service is available for participants who are preparing to live on their own or for those who are in jeopardy of losing their current housing. This service promotes being successful in private homes owned, rented or leased by the participant.
Assistance for transition youth 14-23 in developing the skills they will need to become a productive and healthy adult. These services are provided individually and in groups.
This service is contracted to our agency by the State of Pennsylvania. Designed for Individuals with intellectual and physical disabilities, this service focuses on all aspects of employment, starting from resume building and job finding, all the way to individualized support in the workplace.
This service is designed to assist our Individuals in various areas, mainly focused on obtaining and maintaining competitive integrated employment. The service definition for Supported Employment is broad by design, which allows for the agency to assist in a number of ways for the participant.
Utilized to bring families together with their natural supports, as well as Service Providers, to develop a plan around a specific area of concern. This also includes Youth Transition Conferences.
Identifies a child’s greatest unmet need in the family unit. Once the unmet need is identified this becomes the bottom line for the Family Group Decision Making Conference.
This process is utilized to bring a youth (individual 14 years or older) together with their natural supports to develop a plan for them to transition from one part of their life to another. Many times, this process is used for youth that are transitioning into adulthood from placement. During this meeting the youth and supports discuss goals that should be looked at as they are transitioning. Similar to a Family Group Decision Making Conference, this is one major meeting to bring all supports together at one time. The guest list of invited persons is generated by the youth wishing to have the Youth Transition Conference. There are pre-meetings to prepare everyone for the Youth Transition Conference and a Post Conference Meeting to bring the group back together again to go over the plan and ensure that things are moving forward accordingly.
Utilized to identify natural supports that are available to a child that is involvement with Children and Youth Services and what specific things that support is willing and able to do. Through this process the natural supports are able to identify in what ways they can support the child and family.
Parents as Teachers is internationally recognized evidence based early childhood home visiting model. Parents as Teachers increases parent engagement to promote parenting that fosters children’s development, motivates a commitment to parental growth and behavior change as well as improves parents’ abilities to discover their connection to their child’s behavior and their family’s well-being.
